Terms & Conditions and PDPA :
1. General T & C
  1. The Family Plus Membership Programme (the "Membership") of Pantai Hospital Cheras ("PHC") is governed by these Terms & Conditions.
  The submission of an application for Membership is deemed to be an acceptance of these Terms and Conditions.

  i. Malaysian citizen or non-citizen residing in Malaysia; and
ii. Aged 18 years old and above.
2. Eligibility and Membership Application
1. Family Plus Membership is free and open to all individuals who fulfil the following criteria:

3. Family Plus Membership application is Free but renewal fees may apply.
4. Applicants are required to complete the mandatory registration with accurate information and to confirm acceptance of the applicable Terms and Conditions and Privacy Policy.
5. Upon successful registration, applicants will able to access their Family Plus Membership account and view their Membership details. Members can collect points at PHC ONLY and NOT at any other Parkway Pantai Hospitals. Points collection is not applicable at any participating Merchant Partners outlets, however members will able to enjoy the benefits and privileges offered.
6. Members are required to show their e-membership cards for verification purposes during points collection and redemption at PHC and participating Merchant Partners outlets.
7. Membership account is non-transferable to another individual. The same applies to the accumulated points by member which is non-transferable to another individual.
8. PHC reserves the right to suspend or terminate this programme at any time as it deems necessary. In such a case, PHC shall give an advance notice of 6 months to all members in the manner PHC deems appropriate.
9. PHC may refuse an application for Membership or to terminate an existing membership. PHC reserves the right not to disclose the reasons for such refusal.
10. PHC reserves the right to omit, add or amend the Family Plus Membership Programme Terms and Conditions without prior notice. Any new or amended Terms and Conditions will override the existing Family Plus Membership Programme Terms and Conditions.
11. Whenever there is a change of personal particulars, it is the Member’s sole responsibility to update their Membership record in the Family Plus Member’s Portal at www.familyplus.com.my or email to PHC Marketing Team at [email protected]
2. Points Collection
1. Every One (1) Ringgit Malaysia (RM) spent in Pantai Hospital Cheras (PHC) shall be converted to 1 point.
2. Payments made via Cash, Credit/Debit Card, E-Wallet, Insurance/ Corporate Guarantee Letter are valid for points collection. Payments, purchases and transactions must be fully settled or competed prior to points to being awarded.
3. Points collected is valid for 2 years from date of issuance. Any expired points will be forfeited without any prior notice.
4. Payments made at Cafeteria, Promotional Booths, Retail Tenants and Parking in PHC are not valid for points collection.
5. The points collection entitlement in the Family Plus System is final and no further correspondence will be entertained
6. Family Plus Points accumulated are non-transferable, non-assignable and non-saleable to any other persons.
7. PHC reserves the right to deduct the balance of a member’s points accumulation in the event that:-
  any point is suspected of being fraudulently recorded
any point is recorded in error
any point related to a transaction which has been cancelled or where a refund has been given
upon redemption of points
upon expiry of points
3. Points Redemption
1. Only valid Family Plus Members holding a valid and non-expired Family Plus Membership Account with sufficient points are eligible for the Points redemption.
2. Family Plus Members are required to produce their e-membership card, Identification Card or Passport during the process of points collection and redemption, for verification purposes.
3. Upon Pantai Hospital Cheras’s (PHC) acceptance of the redemption order, Family Plus Members shall not revoke or cancel the redemption without a valid reason, acceptable by PHC.
4. Family Plus Members can log in to www.familyplus.com.my to check their points.
5. PHC reserves the right to refuse any redemption of points if PHC has any reason to suspect the points were fraudulently accumulated by the member or wrongly recorded.
4. General Rules
1. Your Membership account remains the property of Pantai Hospital Cheras (PHC). Use of your Membership account constitutes acceptance of the benefits, privileges and the terms and conditions listed herein.
2. PHC reserves the right to change or cancel Members’ Point accruals or balances, at any time without prior notice and without providing reasons whatsoever.
3. PHC holds absolute discretion on the computation of Points awarded to Members. PHC’s decision on any disputes is final and conclusive.
4. The Points earned structure may be subject to change from time to time as determined by PHC. PHC reserves the right to add, remove, substitute or change the Points issuance, conversion or redemption structure from time to time at our sole and absolute discretion, without prior notice.
5. PHC reserves the right to amend the rules, restrictions, programme benefits and special offers without prior notice. PHC is not responsible for participating Merchant Partners’ withdrawal from the programme or partners’ privilege cancellation, which may affect the rewards offered.
6. PHC reserves the right to terminate any Membership or the Membership Programme at any time without prior notice.
7. PHC assumes no contractual or legal responsibility to Members. In earning and accumulating Points, Members understand that they may not rely on the continued availability of any rewards, privileges or other benefits. PHC will not be responsible for any delay in the posting and/or accrual of the Points.
8. PHC makes no warranties or representations, whether expressed or implied, and expressly disclaim all liability (including consequential damages) with respect to the quality of goods or services claimed, redeemed and/or enjoyed through the Membership Programme. All conditions, warranties or other terms implied by law are excluded to the fullest extent permitted by applicable laws and regulations.
9. All redemption terms are subject to availability and any restrictions that may apply as to where and when the points and rewards can be redeemed. PHC reserves the right to change, replace or substitute the redemption terms without prior notice.
10. PHC assumes no liability and responsibility for any privileges, benefits and/or services which are not available to any Members due to geographical limitation or any other reasons whatsoever.
11. Fraud or abuse concerning the use of Points by any Member shall result in the immediate termination of the Membership and forfeiture of points accumulated
12. Failure by PHC to enforce any particular Terms and Conditions regarding the Membership Programme does not constitute PHC’s waiver of those Terms and Conditions.
13. In the event of any dispute, the Member agrees to submit to the exclusive jurisdiction of the courts of Malaysia.
14. Family Plus is a membership programme operated by PHC. Upon applying for Membership:
  i. You represent that all the information or personal data provided in Membership online registration is true, accurate and complete;
ii. You consent to PHC’s collection and use of the data required for the Membership. PHC is committed to maintaining the confidentiality of your personal data;
iii. You consent to PHC in updating you with information such as but not limited to promotions, healthcare news, health talks, health and wellness events via direct mails, emails and text messages; and
iv. You accept all the terms and conditions contained herein and in connection with your Membership.
15. You must use all reasonable efforts to keep your username and password confidential. You are solely responsible for each and every access or use of the Membership site at www.familyplus.com.my that occurs in conjunction with use of your username and password.
16. You must not allow any third party to access or make use of the site using your username and password. You must notify PHC in writing as soon as you become aware of any unauthorized use of your Membership Account, username or password.
17. PHC is not responsible for any harm that you or any person may suffer as a result of a breach of your confidentiality in respect of your use of the Membership site. While PHC has taken precautions to protect information that you transmit to the website over the internet, PHC does not warrant the security of that information. Once information is received, PHC will take reasonable steps to preserve the security of that information.
18. By participating in PHC events and/or accepting any promotions, privileges and/or benefits offered by PHC and our Merchant Partners, I hereby give my consent to any photography and/or video recording of me which can be used by PHC in any form of printed materials, electronic and digital media formats for any purposes including but not limited to promotional and commercial reasons. I understand that the images and/or videos containing me may be stored and retained permanently in PHC archive.
19. PHC reserves the rights to revise the above terms and conditions at any time without prior notice.
